Bree began Wise Decisions after having had the personal experience of needing a "boots on the ground" supportive advisor for her ailing father with who lived in West Michigan and desperately needing a local advocate after his multiple hospitalizations.

She could not obtain a community based Social Worker or senior advocate through any agency to offer care transitions or relocation planning assistance, so she downsized her job to have flexibility to travel from Texas once a month to be his advocate until he agreed to relocate to Texas one year later.

Having done this type of care navigation for many patients in Texas for nearly two decades, it was obvious that there was a need that she could fill for  many other long-distance loved ones who may find themselves in the same position.  

Wise Decisions Care Management was founded in 2025 after Bree transitioned out of Hospital Complex Comprehensive Medical Care Management and 20+ years of Home Health, Palliative, and Hospice Social Work.
